PROCEDURE <br /> <br />A qualified health care professional can review an inmate’s arrest and confinement <br />record for information relevant to the inmate’s care and treatment. <br /> <br />There will be a field memoranda at each institution that identifies those w ho have <br />access to those records. <br /> <br />Maximum communication and cooperation with thorough documentation of this <br />communication between prison administrators and medical support services are <br />required. When high risk and/or special needs patients require movement, changes of <br />assignments or transfer, notification will be a priority. <br /> <br />If there are circumstances of an inmate’s health status which justify advising the warden <br />or designee of an inmate’s condition (need to know), this will be coordinated through the <br />facility health authority or designee. <br /> <br /> <br /> <br /> <br /> <br />___________________________________________ _____________________ <br />Administrator, Institutional Services Division Date
