Submit Appendix A, Medical Request for Payment <br />Authorization, to the Health Services staff. <br />3. Offender Concerns <br />Offenders who feel as though they have been wrongly denied a requested healthcare <br />service shall have the right to file a concern, which must be done by completing an IDOC <br />Offender Concern Form and submitting it to the proper authority for resolution. (See SOP <br />316.02.01.001, Grievance and Informal Resolution Procedure for Offenders, for procedures <br />and the concern form.) <br />4. Compliance <br />Compliance with this SOP and all related Department-approved protocols will be monitored <br />by the health authority (or designee) by using various sources to include: this SOP, clinical <br />practice guidelines, routine reports, program reviews, and record reviews. <br />The health authority (or designee) must conduct two (2) audits a year (or more frequently as <br />desired based on prior audit results).
